You can perform a counterattack immediately after blocking an enemy attack.
Guard counters make it easy to break an enemy's stance.
The PlayStation and Xbox buttons below use the standard controller layout. If you changed the controls, use your assigned buttons. On PC with keyboard and mouse, open Main Menu → System → Key Settings → Keyboard/Mouse and check the bindings for the actions described below; do not assume controller labels are keyboard keys.
Block an enemy attack, then immediately use Strong Attack (right-hand or two-handed armament): PlayStation R2; Xbox RT. This is not the Skill or normal Attack action. A following enemy hit can interrupt the counter, so a successful block does not guarantee a safe opening.
